The toes are intentional. The only problem is I'm missing the pin that holds in the bolt catch, so I can't QUITE finish it. (The area where the bolt catch should be is circled in blue) All this means is the bolt won't lock back when the magazine is empty. I should still be able to shoot it without a problem. I am excited!

We were at a pretty far distance for pistols... so that probably has something to do with it. I know the wind wasn't affecting it at all, really... I was just trying to come up with some reason for it.

Improper manipulation of the trigger. It's very easy to push the trigger to the left or pull it to the right (or the other way around if you're left handed) when you pull it. 2. Twitching in anticipation of recoil. From what Chris tells me, that doesn't seem very likely... 3. The sights aren't zeroed. The sights on an XD aren't really adjustable, so this would be the worst case scenario...
